Eye Exam Required by State of Illinois New This Year

Kindergarten, ECE and Pre K Students

The state of Illinois is now requiring all new Kindergarten students and/or any student entering school for the first time to provide proof of an eye exam.  This does include ECE and Pre K students.

An eye examination by an ophthalmologist or an optometrist is required for students entering Kindergarten or any Pre Kindergarten (within 1 year prior to entry). Proof of the eye examination on the approved State of Illinois Eye Exam Form with the signature of the eye doctor is required by October 15, 2008. These are included in the registration packets. The state of Illinois does provide an eye examination waiver form available to families experiencing financial hardship.  This form is available in your elementary school office.

The McLean County Health Dept has an eye clinic available to children in McLean County who meet income criteria. Call 309/888-5479 for more information. Students who qualify for eye care through the Illinois Dept of Public Aid are not eligible for the MCHD eye clinic. Students that possess a Public Aid medical card may call Scott Health Resources at 309/823-4224 for a referral if needed.
